With A Chronic Disease Management Plan (CDMP) from your GP –

  • Initial consult (45-50 minutes) : $95 ($90 per pension/concession) -rebate $62

  • Review consults (30 mins): $68 ( rebate $62)

  • Clinics available currently- Currambine, Butler , Joondalup, telehealth

    Private Fees: if you don’t qualify for a Chronic Disease Management Plan

  • Initial $100 (45-50 min), $95 (pensioner/ concession)

  • NDIS $150 per hour of direct or indirect client time

  • Initial extended (60 mins) $115 (Bariatric, disordered eating) (60 min)

  • Review $70 (30 minutes), $65 concession

Rebates: Private Health (ancillary cover) may cover a portion of the cost if you have cover for Dietetics. Check your cover before coming along. Claiming available in the clinic so you will only pay the GAP

The one-on-one online consultation that kick starts this program ensures that I fully understand your lifestyle and nutritional needs. My plan for you will be realistic and sustainable, because this isn’t about short-term perfection, it’s about long-term life changes that you actually enjoy implementing! My ongoing support is unlike any other program – if you have questions or concerns you can ask me directly and have them answered quickly.

After the program finishes you can choose to take what you learned and apply those skills going forward

  • Initial video/phone one-on-one consultation

  • Detailed personalised nutrition PDF report

  • Recipes

  • Support via phone, text, email or Facebook as required for the entire 6 weeks.

  • weekly check ins

  • 1 follow up video/phone consultation at week 4

  • Skills and knowledge to take with you beyond the 6 weeks program.
